Key Diagnostic Tests for Infertility

We use advanced diagnostic tools to uncover the underlying causes of infertility in both partners. Common tests include:

  • Semen Analysis – To assess sperm count, motility, and morphology in the male partner.
  • Sperm DNA Fragmentation Test – Recommended for men with poor sperm parameters or unexplained infertility.
  • Transvaginal Ultrasound – To check the uterus, ovaries, and pelvic structures; and estimate ovarian reserve (Antral Follicle Count).
  • Hormone Testing – AMH (Anti-Müllerian Hormone, the most important marker of ovarian reserve), FSH, LH, Prolactin, and TSH.
  • Hysterosalpingography (HSG) – To check if the fallopian tubes are open and functioning.
  • Operative Laparoscopy & Hysteroscopy – To treat or correct conditions such as fibroids, endometriosis, polyps, adhesions, or blocked tubes.

Common Infertility Diagnoses

1. Tubal Factor (Blocked Fallopian Tubes)

When the fallopian tubes are blocked, eggs and sperm cannot meet.

  • If a hydrosalpinx (fluid-filled tube) is present, it must be removed or clipped before IVF to prevent embryo washout and repeated IVF failures.

2. Endometriosis

A condition where endometrial tissue grows outside the uterus, often causing cysts and adhesions.

  • First-line treatment is laparoscopic surgery to remove cysts and adhesions.
  • Couples may then try naturally or opt for IUI.
  • If these fail, IVF is recommended, especially if the ovarian reserve is low.
  • For moderate to severe endometriosis, we may freeze embryos before surgery to preserve fertility.

3. Polycystic Ovary Syndrome (PCOS)

PCOS affects ovulation and hormone levels.

  • Initial treatment includes ovulation induction and IUI for a few cycles.
  • If unsuccessful, IVF with modern protocols and frozen embryo transfer offers up to 60% pregnancy rates in two cycles, while avoiding Ovarian Hyperstimulation Syndrome (OHSS).

4. Male Factor Infertility

Low sperm count, poor motility, or abnormal morphology can hinder conception.

  • Tests include hormone analysis (FSH, Prolactin, Testosterone), scrotal ultrasound, and Doppler studies.
  • Treatment may involve medications, lifestyle changes, or surgery (e.g., varicocelectomy).
  • If sperm quality remains low, ICSI (Intracytoplasmic Sperm Injection) is the most effective treatment.

5. Unexplained Infertility

When all test results are normal but conception still doesn’t occur, it is classified as unexplained infertility.

  • Possible subtle egg, sperm, or fertilization issues may exist.
  • Treatment often starts with 2–3 cycles of IUI, followed by IVF or ICSI.

6. Repeated IVF Failures / Implantation Failures / Miscarriages

For women with recurrent IVF failures or pregnancy loss, advanced therapies can help:

  • Hysteroscopy with endometrial scratching
  • Frozen embryo transfer with high-dose estrogen
  • Use of medications like sildenafil, aspirin, heparin
  • Intrauterine instillation of growth factors (G-CSF or PRP)
  • Gestational surrogacy may also be an option in select cases.

7. Poor Ovarian Reserve

Low egg count or quality, more common after age 34 but can occur earlier.

  • Diagnosed via low AMH levels and low antral follicle count.
  • Treatment includes pre-treatment with androgens & supplements, followed by IVF with embryo accumulation over 2–3 cycles.
  • Ideally, at least three high-quality embryos are frozen before transfer.
  • If unsuccessful, egg donation may be considered.
  • Dual stimulation (two stimulations in one cycle) is no longer recommended except for urgent cases like cancer.
